6.11.3 unable to create / edit VM

All I see is the word "Icon" and the icon of the VM when I try to add a new VM or edit exiting VM. Tried both Safari and Firefox just in case it was a browser issue, no difference. I am able to start the VM.

    Chrome (Android)
    1. Tap the lock icon next to the address bar.
    2. Tap Permissions → Notifications.
    3. Adjust your preference.
    Chrome (Desktop)
    1. Click the padlock icon in the address bar.
    2. Select Site settings.
    3. Find Notifications and adjust your preference.
