Solutions

  1. holtdw01's post in 2nd flash drive failure in one week (the odds don't support this happening) was marked as the answer   
    thanks all, pulled usb drive and ran surface scan, no errors found flash drive still good. Recopied config from backup (in case something got corrupted). Reinserted, booted up and and all is good.
