Disk4 was assigned in the array, and if you formatted it then it is still assigned in the array, so I don't understand what you mean by "not added to the array". Maybe a screenshot will clarify. And of course, if you formatted the disk then any data on it is lost. Your "solution" is not the way to proceed if you want to keep the data that was on the disk.
