Overnight I got a bunch of warnings, Disk with read error.  when I ran a smart short self-test the errors appeared again in the logs and the

Raw read error rate had increased

If you mean disk5, yes it appears to be failing, but there's no recent SMART test, run an extended test.

Running extended test now.  will repost diags once complete

Finally finished extended self test.  ended with no errors.  

 

So is disk 5 still hosed?

SMART test passed so disk is OK for now, but you want to keep an eye on these attributes:
