I swapped SATA cables and moved the mobo port but still no dice. It's failing to initialize, currently it's on the Intel SCU controller, these are known to not be as rock solid as the standard SATA controller, if not yet done try swapping with a disk on the Intel SATA controller, if the same happens there it could be a power or disk problem. 1 Quote Link to comment

I ran smartctl -a /dev/sdv and the output is pasted above.
