Unable to Assign Drives/Drive Disappeared after attempting to start Array

I have been working on trying to do my first UnRaid install/setup. So far, I have been able to get everything up and running, however when I try to add drives, I am running into issues where no drives appear in the GUI. All drives appear as SCSi devices when checking for system devices, however when I try to assign them, they are missing. I was able to get one drive to show up, however after attempting to start the array, it didn't work and the drive is no longer able to be chosen to assign. 

 

Hardware is a HP ProLiant ML 330 G6 with a SmartArray P410i controller. I have tried configs with both 3 drives in separate RAID 0, as well as all three drives in a single Array (just to see if they would show up).

 

I've also attached the diagnostics from the UnRaid instance. 

 

Any help is greatly appreciated!
