Failing Drive?

I think I have a failing drive, but I'm not really sure how to interpret my SMART results and need a little help knowing how to proceed. I'd like to find out if my understanding is correct that my drive is failing and I need to replace it, and whether or not my issue represents a case where I could take advantage of a warranty or if this isn't considered bad enough for a warranty replacement. I do have a valid warranty, but don't know exactly what I'd need to do to take care of the issue if I need to replace the drive and would like to understand exactly what's going on with it.

 

When I logged into my webGui today, I had several notifications indicating 100 reallocated sectors, 94 reported incorrect sectors, 100 current pending sector, and 100 offline uncorrectable on my parity drive. I also have a yellow thumbs down next to the drive in my drive listing on my dashboard under the smart column. My last parity check was 5 days ago with no errors, so unless I'm misinterpreting something, this is new. I am including my diagnostics file as suggested in the "NEED HELP? READ ME FIRST!" post.
