Drive read/write error during monthly parity check disables drive

Hello, my disk 11 is disabled while conducting the parity check at the beginning of the month. This is the second time since August this has happened. Both times has been disk 11. I have replaced the drive, even though it was a new drive. I also replaced the SATA cable and used another SATA port on the motherboard. At this point I am looking to remove the drive all together and shrink my array size. I have pulled a diagnostics package and attached it here. Any ideas to what could be the cause are much appreciated and as to possibly retrieving the data (2.03GB) that is on the drive if I shrink the array by removing the drive. Thank you.

disk11 has disconnected so can't see SMART for that disk, but I suspect there is nothing wrong with it or the original disk11.
