Unable to start array.

Unable to start array with new disk. The disk originally failed and i replaced it with a new disk. I check all cables and power connections. Precleared the disk and all worked. Attached diagnostic logs.

Sorry for the delay. I fixed it.... well i thought i did but it came back on a different disk.

Disk dropped offline, check/replace cables and post new diags after array start.

Performed those actions everything went ok till today. It disabled the same disk again.

Server wasn't rebooted since last time, so cables were not replaced, did you at least check them? Though not easy to really do that with the server on, swap cables/slots between that disk and another one then see if the problem follows the disk or not.
