Struggling to get my PCIe NVMe card to work

I recently purchased a IO CREST SI-PEX40152 (Supports non-bifurcation motherboards) and I'm not sure what I'm doing wrong? I've been doing 50 hour weeks recently, so apologies if I just didn't research the issue properly.

I've tried installing it in both my Windows 11 PC and my server, and it doesn't seem to be showing up in either device list (I've attached the unraid HW profile below), but apparently this is supposed to be pretty plug and play. 

The only thing I can think of is that it's DOA, it is refurbished afterall, but I'm trying to exhaust my options before waiting forever for the refund + new item process.

There are two NVMe device, how many are there supposed to be?

Those are the ones directly installed to the motherboard, the PCIe card has four

Not seeing anything else detected, so likely a hardware issue.

Ah, that sucks, I'll try to look at the hardware one more time, but it's probably just DOA.
