I rebooted and my server is now saying to replace media and hit any button to restart.  

 

What is the process to recover the USB drive?  I can build a new usb drive but which folders do I need to copy from the old to the new?

    It turned out that a bad drive connected to my SAS controller was preventing the USB drive from booting.  I'm not sure why this was happening since the SAS controller shouldn't have been a bootable device.
