Unraid [6.12.4] unexpected shutdown when running plex

My Unraid server keeps unexpectedly shutting down.  After clean restarts, array rebuilds and all sorts of hardware troubleshooting, I've come to the conclusion that the issue must reside in unraid or the official plex docker app.  I've setup the syslog server but have no idea what I am looking at, can anyone with more expertise help me understand what might be the reason for the crashes and shutdowns?  I've seen the console output "BUG: Bad rss-counter state" but that's about all the info I have.

in addition, when I restart the Unraid server and try to start plex I get this message in the plex logs on repeat:

 

Error: Unable to set up server: sqlite3_statement_backend::loadOne: database disk image is malformed (N4soci10soci_errorE)
Stopping Plex Media Server.
Starting Plex Media Server.

If the server really shuts down (vs crashing or hanging) I would suspect a hardware problem.
