My Disk1 went offline last month, but I was able to remove/re-add the disk and rebuild the array. It has gone offline again. SMART looks ok. Is there anything I should be checking to ensure this doesn't continue to happen?

    It's not logged as a disk problem, try replacing/swapping cables/slot to rule that out, and if possible I would use the onboard SATA ports instead, then rebuild.
