Unraid freezes after about 5ish days

I've been chasing this issue for a two weeks now. After an amount of time running varing between 3-5 days it seems, unraid will freeze and do one of two things. Either I get an nginx error trying to access the GUI or nothing responds at all. Recently it has been completely unresponsive. I cant even SSH into the machine. The hardware is a Dell T420 that has been my machine for about 3 years now without issue. 

To try and dig in, I set the syslog to write to flash. Below is the most recent log. Looking at when my docker container for Unifi died, it appears to have gone unresponsive around 10:20-10:25am. I cant seem to find any evidence that would point to the issue and I'm at a loss. Unraid is on 6.12.4 and has been for a while now. Should I attempt an upgrade up? Any help is greatly appreciated!

Be sure all yur plugins are updated.  Some are very old.

 

Be sure your Docker Custom network type in Docker Settings is set to ipvlan.

I updated the plugins and OS along with most of the docker containers. Thanks for the help and I will update in the future!
