Repeatedly crashing when upgraded to 6.12

I initially tried to upgrade my OS months ago and noticed some serious stability issues. A few weeks ago I started to notice warnings on how the Community Applications plugin could not upgrade. I upgraded last weekend and my server has crashed 3 times this week. I had to run XFS repair on all drives each time with the -L and parity check. I rolled the OS back and now cannot even install the Community Applications plugin which is not ideal. Anyone have any idea what is going on? I have attached the diagnostics that were run as I rolled back the OS.

Those are just after boot and without the array started, so aren't telling us much. Setup syslog server.

 

Why do you have 100G docker.img?

Thanks for getting back to me. I have a 100GB Docker Image as I had a bad configuration which was causing it to grow like crazy but haven't downsized it yet after fixing it like 6 months ago. I have setup the syslog server but am probably at an impasse for now as I have no intention of upgrading again and dealing with it anytime soon. Thanks for trying to help though.
