Looks more like a power/connection issue, but since the disk dropped offline there's no SMART, check/replace cables and post new diags after array start.

    Got it. I am rebuilding that drive now and have ordered a new case. I checked all of the cables in this one, Norco RPC 4224, and everything was solid. Perhaps the backplanes are going. I already lost one of the 24 connectors several years ago. Not sure how or why.
