OCZ Vertex 2 SATA SSD is not showing in UNRAID (does show in BIOS)

Hi, I'm new to unraid, just setup up my new system, but I have a really old SSD which works perfectly fine in my old PC, it's OCZ Vertex 2 60GB SATA SSD, the experience is very unpredictable, the first time I boot it, it didn't appear in unraid, so I thought maybe it's the NTFS is not so friendly to UNRAID? I even downloaded the "unassigned devices" plugin as suggested by my friend, still not able to see it (although it definitely shows up in BIOS every single time), so I pulled out my windows installation USB drive, completely removed the partition, now it shows up in unraid for the very first time! HOWEVER, after a reboot, it's gone again. I spent hours trying different things, nothing helped. Has anyone experienced this before? Any sugguestions are much appreciated. Diagnostics report is attached. Thanks!

Not sure what is going on if you can see the SSD in the BIOS as there is no sign of it showing up even at the hardware level as Unraid is loading.   Any existing content/format should be irrelevant.

I guess it probably reached the end of its useful life, I'll just replace it, this is too unstable to use at this point.
