Unable to assign new array drive, system keeps clearing the assignment

The system notified me last week there was a smart error on Disk 4 of my array.  It automatically disabled the drive.  I shutdown my system, installed a new drive (of the same size), and precleared the new drive without issue. 

The new drive shows up as an option for Disk 4 but after selecting it, the screen flashes and the assignment is cleared. 

 

Any help would be greatly appreciated.

You should post your system's diagnostics zip file in your next post in this thread to get more informed feedback.  It is always a good idea to post this if your question might involve us seeing how you have things set up or to look at recent logs.

Thank you for responding @itimpi, good point. 

I think my issue is now resolved.  Turns out my Flash drive was read-only.  I wasn't able to run error check via Windows (I kept getting the device is write protected), so I followed the instructions for replacing my Flash drive (restoring from a backup & transferring my license). 

I was then able to select the replacement drive, start the array, and am now waiting for the rebuild to complete.  🤞
