Problem with unmountable drive

Hi, 

I recently added this new drive (disk 5) to my array and after a few months it gave me an error and ended up not being able to mount.

 

I have no idea what to do. I tried new config, but no luck.

 

Any help would be greatly appreciated.

    Check/replace cables for disk9 and post new diags after array start.

    3. Adjust your preference.
