Drive rebuild keeps crashing

I am trying to upgrade a drive and when I start the rebuild it goes for a while and then stop between 1 and about 5% complete.

 

I had a sata card in the system but read that could be a problems so swapped it out for HBA card (LSI sas9300-16i), performance has improved but it is still crashing before the rebuild completes. I have tried with 3 different hard drive and it is still crashing. Attached in the latest diagnostic file. Any help would be greatly appreciated as I am been fighting the for several weeks.

The syslog in the diagnostics is the RAM copy and only shows what happened since the reboot.   It could be worth enabling the syslog server to get a log that survives a reboot so we can see what happened prior to the reboot. The mirror to flash option is the easiest to set up, but if you are worried about excessive wear on the flash drive you can put your server’s address into the Remote Server field.

HBA problems, make sure it's well seated and sufficiently cooled, you can also try a different PCIe slot.
