Disks Disconnecting and reappearing, I've tried everything.

 I've replaced all old drives, SAS breakout cables, replaced the power supply and power extenders, reseated the HBA controller.

 

Random drives drop off and different times, brand new drives both SSD and HDD, some with less than 4 days on them.  I can find no common denominator.   I can move cables from a drive showing no issues to a drive with issues and have them both work fine, reboot and another random drive falls off. 

 

I'm at a loss here and have no idea how to proceed.

Diags are after a reboot, so we can't see what happened, though it looks like you have already tried everything, but still worth posting them.

I have a similar issue that leads to read/write errors only on a specific drive point in unraid which then gets disabled (Like you I have replaced PSU, SAS cables, removed SATA extenders and checked and reseated HBA controller) and thinking there may be a bug involved as the former physical drive and the current one don't show the failure as drive related and work/test fine in other non-unraid machines, as does the HBA controller
