HBA issues, make sure the controller is well seated and sufficiently cooled, you can also try a different PCIe slot if available.

    When I went to reseat the HBA, I noticed it was touching the CPU heatsink. I also flashed the HBA's firmware about a week ago. I'm hoping the former is the issue and not the latter. On reboot, the array returned to normal.
