Custom VM Template Stuck in XML Only

I'm currently trying to install a custom VM however the template is stuck in XML mode with no switch to revert it to form. I've cleared cookie caches and tried multiple browsers / devices but all result in exactly the same issue. I'm aware of the cookie vmmanager_listview_mode, however removing/editing does not cause the toggle switch to re-appear, let alone change.

 

Has anyone else experienced this?
