"There is a physical problem accessing your USB Flash boot device" Out of ideas

Featured Replies

History:
I have been getting this error for quite some time all the way back in version 6. Was working fine in this configuration for quite a long time then just started erroring out. I get the banner in as soon as an hour or sometimes days. Lately it errors out in under 24 hours just about every time.

Things I have tried:
Multiple USB sticks, swapping to every port on the motherboard and even changing out the motherboard. The problem persists and I am at a loss on what to try next?

Flash drive is dropping offline, if you have already tried all the USB ports and different devices, there may be an issue with the board/USB controller.

P.S. there appears to be a container constantly restarting, see if you can find out which by looking at their uptimes.
