HBA problem, make sure it's well seated and sufficiently cooled; you can also try using a different PCIe slot.

Oh man, thank you very much!

Did a rebuild-in-place, worked for a day... guess the HBA got jiggled in just the wrong way again and POOF... emulated disk.

Will post resolution ASAP!
