5.0-rc4 array fault randomly

I am having an issue where randomly I get an array fault, to fix it I can shut down the machine and boot it back up and everything is fine again although it sometimes means it rebuild parity.  When it does happen I am seeing a lot of errors (30k errors sometimes) on disk 2 which seems to be invalidating the parity.  SMART reports come back clean with no issues on all drives.  Its almost as if the system is no longer even recognizing disk 2 or the parity drive disk 0 until I reboot the system.

 

Anyone have any ideas on what it might be?  I've included my syslog.  Everything will run fine for 2 - 7 days and then the problem happens again.

 

The log errors look like this:
