could someone take a look at this report and give me some idea of whats going on the drive has failed but not sure why can I just format it and put it back or do I need to send it back?

The errors could be cause by a bad or loose cable or SATA port. Or EM interference on the cable. The SATA port on the drive itself could be flakey. I'd run pre-clear on the drive and the use it.

That definitely sounds as if the drive failure is likely to be terminal.

as its just 2 months old will download their testing tool and test the drive and do a return. off shopping for a replacement to get the array back up and running and fully protected again.
