Did the drive pass at least one preclear pass before you added it? New drives are far from a safe bet, quality control is pretty bad. http://en.wikipedia.org/wiki/Bathtub_curve
December 9, 201411 yr Author yes made sure I did preclear script before I added this drive to array it has also now passed short and long smart tests
December 9, 201411 yr Were you physically moving the drive and have you checked for bad SATA cables? A momentary disconnect in the cable can lead to a red balled drive. I would check SMART reports and replace the SATA cable. If you see no problems in the SMART report, you're probably OK re-adding the drive to the array and seeing if you have a problem again.
