faulty disk?

Did the drive pass at least one preclear pass before you added it? New drives are far from a safe bet, quality control is pretty bad. http://en.wikipedia.org/wiki/Bathtub_curve

yes made sure I did preclear script before I added this drive to array

 

it has also now passed short and long smart tests

Were you physically moving the drive and have you checked for bad SATA cables? A momentary disconnect in the cable can lead to a red balled drive. I would check SMART reports and replace the SATA cable. If you see no problems in the SMART report, you're probably OK re-adding the drive to the array and seeing if you have a problem again.
