M1015 Detection and Flashing

Guys, I recently purchased a M1015 to replace my existing 4port controller card because I've been experiencing a bunch of issues with it.  I'm trying to flash it right now. 

 

I installed it into my HTPC (ASUS P8H61-I) but it doesn't seem to be detected.  Nothing in Device Manager, during the POST, nothing at all. I tried running the megaraid utility to search for adapters and it doesn't find anything.  I did some searching and discovered that 1) the card has LEDs and 2) another user that attempted to use the same card ran into similar issues and resolved it by flashing his BIOS.  I decided to skip that and just throw it into my server to get it up and running because I know my server is on the latest BIOS.

 

My server uses a Supermicro X7SPA-HF.  I installed the card as normal but once again, nothing at all.  No lights on the card or anything too.

Needless to say, I'm extremely frustrated and at a complete loss.  How do I proceed?  Thanks for any and all help, as always.  Is it DOA?

Do you have any other computers to test the card in? When I was trying to flash my card I had to pull out an old AMD motherboard. My ASUS board didn't detect the card until I flashed it to IT mode. You should have a green blinking LED and 2 solid green LEDs. Card might be bad.

Try the EFI method in the M1015 tools link in my signature below.  On my Supermicro X9SCM, the full EFI shell is the only way to have the card show up.
