Any ideas where the database is stored and why it is not persistent or what can be done to make it persistent?

I see you are using Phazes plugin.

 

Create a cache only directory.  I have a directory called "apps" that I use on my server.  On the plugin configuration page set your settings as appropriate.  Below are my settings.

 

Config / Library directory: /mnt/cache/apps/PlexMediaServer

Transcode directory:  /mnt/cache/apps/tmp/plex

 

 

Under the information area, you will get the following messages:

Your config data will persist after a reboot

 

Your trancode directory is located on a physical drive
