Can you put your flash drive in a windows machine and run a checkdisk,?  Make sure you shut down the system first.

Just after setting up your User Shares for export, the restart of SMB appears to fail, don't know why.  The only thing that is slightly different to other syslogs is that your shares are capitalized, which should not matter.

A few other oddities or issues -

* You received a read error on Disk 2, the Samsung, but the errors are all a generic "device error", not a bad sector.  Drive is probably fine.  Error could be almost anything, perhaps from the controller, and there is no further information about what was actually wrong.  Could be a power issue ...

* There's a low speed (SATA link up 1.5 Gbps) strange device connected to the last port on the 8 SATA port card, called 'MARVELL VIRTUALL', an ATAPI device, not a storage device.  It is set up, but has one hiccup, a "HSM violation".  A reset seems to make it happy.

* This appears to show that there are still vestiges of v5 on your flash drive, which could cause strange issues.  If you can't find any other reason for the SMB issue, you might want to redo your flash drive.  We strongly recommend a re-format when preparing for v6.  I think you may find the instructions in the Upgrading to UnRAID v6 guide to be useful, in re-preparing a v6 boot drive.  The no-format section shows what can be kept and what HAS to be removed, but recommends re-formatting for max safety.
