Drives continue to red-ball

Hey all, I have a system that has been upgraded and working great for years.  Put some new drives in it and also upgraded to v6 a while back and have had a lot of issues with stability compared to the previous version of UnRAID... :-\

Sadly I had a catastrophic data loss around 6-8 months ago and am very nervous to trust UnRAID again (even though it has served me flawlessly through a fire, hardware upgrades, and constant use.

 

Hope the community can help me decipher the code above and know why my drive (again) was red-balled.

The Diagnostics zip will give a more complete picture, but even from the small excerpt above, there are 2 problems visible - a hardware issue (drive associated with ata3 is loose, either from vibration or loose SATA or power connectors) and a software issue (file system corruption in the BTRFS of Disk 1).  Need the Diagnostics to identify the drive with the loose connection.
