Rebuilt a drive, now showing unmountable

Hello all,

 

I have a 6.1.3 system where a drive showed errors.  It was my last 1.5TB drive so I replaced it with a 2TB matching the rest of my array.

 

Afterwards, the new 2TB disk shows unmountable.

 

I followed the process to check the disk through the GUI.

Spun down the array, put into maintenance mode and ran the checks,

 

Then followed the steps to --rebuild-sb per the output. 

 

I am attaching the syslog
