[SOLVED] All sata card disks have errors

A disk (not running through the sata card) failed the other day. I shut down and ordered a replacement. It arrived, I ran it through two pre-clear cycles, then added it to the array. During the rebuild, disks 6-10 all became disabled. These are all connected to a SUPERMICRO AOC-SASLP-MV8 PCI Express x4 SAS RAID Controller. It did complete, but those other disks...

 

I decided to try another SASLP card I have, but the same error occurred and now just a single disk is reporting as disabled. I'll attach both syslogs. I'm currently sitting in maintenance mode.

I have a couple of those cards and had problems with them that turned out to be due to the fact that they were not perfectly seated in the motherboard slot.  This can easily happen if the backplate is not at exactly the right angle and is applying some pressure on the board.  They would work most of the time but then suddenly (I assume due to vibration) the controller would be temporarily lost and all disks on it would apparently disappear.  On a reboot they would all reappear.    Since I have sorted the seating issue the controllers have proved rock solid.
