At some point there was a write error and unRAID disabled the disk, diagnostics are after a reboot so no way to tell what happened, the disk itself look ok, so you can check/replace cables and try to rebuild to the same disk.

The disk6 content you're seeing now is from unRAID emulating the disk using all other disks + parity, that same content is what you're going to have after the rebuild.
