Last night my drive went to disabled.  I cannot find what the problem is.  I have tested the connections and no problems found.  I have attached my Diag file. 

 

Any help on how to fix this would be greatly appreciated.

I don't see the write error that caused the disk to be disabled. You should try to capture your diagnostics before rebooting since anything that happened before the reboot is lost.

Server was rebooted after disabling disk3, so we can't see what happened, SMART looks fine, so you can try and rebuild to the same disk (you can also do an extended SMART test before rebuilding).
