You should try to capture your diagnostics before rebooting since anything that happened before the reboot is lost. May 23, 201610 yr Community Expert Server was rebooted after disabling disk3, so we can't see what happened, SMART looks fine, so you can try and rebuild to the same disk (you can also do an extended SMART test before rebuilding).
