New disk not showing as available

I've got a new 4TB disk that I'm trying to get cleared and added to my array.  Running 6.2 stable.

I previously attempted to add to the array, when unraid was clearing the disk it failed and ever since the disk is not visible.  I've connected the disk to other computers that can see the disk just fine.  I've ran a clean command through DiskPart on my Windows machine successfully. 

I put it in my server and it's still not visible.  I've added to the motherboard ports, and to my Dell H310.  Any ideas?

I'm not seeing anything in the logs that would indicate an issue. 

Thanks for any help you can provide.

    Disk is not being detected by linux, check cable/controller/enclosure.
