what causes cache disk to become unmountable?

After i set my cache disk to xfs and reformatted it later it eventually went back to unmountable for some odd reason. is there any reasoning behind this?

    Without diagnostics all you can do is speculate, but my first guess would be a hardware error or forced shutdown with the drive still mounted.
