The original errors were Disk 3 in Error State Array has 1 disk with read errors Array Health Report Fail I think this is the specific error from the SMART report, but am not sure scsiModePageOffset: response length too short, resp_len=47 offset=50 bd_len=46 scsiModePageOffset: response length too short, resp_len=47 offset=50 bd_len=46 >> Terminate command early due to bad response to IEC mode page A mandatory SMART command failed: exiting. To continue, add one or more '-T permissive' options. I'm posting since I tried searching the forum but would like to be careful so I don't mess anything up since this is my first time troubleshooting this type of problem. December 21, 20169 yr The SMART report for the 3 TB WD disk shows that it has dropped off line. The syslog is full of errors pertaining to that disk and shows nothing useful. Shut down and check the cables to Disk 3, then power up and grab your diagnostics again. The aim is to get a SMART report for Disk 3.
December 21, 20169 yr A little more information. The disk controller has lost communication with Disk 3. That could be because the disk has failed or the controller port has failed or, most likely, that there's a cabling issue. It has been bad for several days (since before 13 December) and your syslog is full of error messages about it. The earlier part of the log has been rotated out and deleted to save space. If you had spotted it earlier you might have seen some useful information. Do you have alerts enabled? You should. It's possible that the controller did a hard reset on the SATA link to try to regain communication with the disk but eventually it gave up and the disk dropped off line. That's the reason for the read errors and the lack of SMART report - compare it with the report from one of your other disks. If the disk is OK then power cycling might well bring it back online but while the server is shut down it's a good idea to check the SATA and power cables to that disk as they are often the cause of problems like this. Replace the SATA cable or at least re-seat both ends. Your other disks look good. If the SMART report looks good you can rebuild Disk 3 onto the same physical disk, otherwise you'll need to replace it.
December 21, 20169 yr Author Thank you! I powered down, made sure the cables were connected properly, powered up and got a new diagnostic report. I appear to have a SMART report for the drive now. This appears to have no errors? tower-diagnostics-20161221-1244.zip
December 21, 20169 yr Yes, that disk looks fine. You can rebuild Disk 3 back onto it. Unallocate the disk from Disk 3, start the array, then stop the array, then reallocate the same disk to Disk 3, then start the array and let it rebuild.
