I have a HDMI KVM switch. My USB is passed through to a Windows 10 VM. Once I switch away from Windows, when I come back, the USB no longer functions. For real machines this works fine.

You could try the hotplug plugin and see if that works. If not, you need to pass through a whole USB controller and then connect the KVM switch to one of the ports.

This is correct.  When you toggle your switch, you are essentially disconnecting the M&K from your VM and hot attach is not directly supported yet. The plugin referenced is probably a good solution but to be truly comparable to bare metal, controller pass through will be needed.
