Post your diagnostics but this is usually the result of a corrupt libvirt.img

how do I create a new libvirt.img?

On the 1st screenshot "libvirt storage location" you need to select the img, not the folder like it's currently.

There no longer is an image to select.  How do I create a new libvirt.img?

write the name in the path, e.g.:

 

/mnt/cache/libvirt.img

 

And click apply.

Done.

I found this thread and tried the solution with no effect.

You need to specify the libvirt.img location not an ISO
