Way too many reallocated. Since you let it get so bad it makes me wonder about the state of your other drives. And you really should have given us complete diagnostics anyway.

At same point in the past disk reached 65C, that's 5C over max operating temperature and way too hot.

The other 8TB Seagate reached 67C, they aren't going to last long at those temps.

No, but I just went in and enabled system notifications.  What should I look for?

The point of Notifications is for unRAID to alert you when you have problems. You should have it setup to send an email or some other form of unattended notification, so you will be informed when things are going bad. If you let things go too bad you may not be able to recover.

 

Do you have a replacement disk?
