Disk with errors (but not ejected)

One of my disks in the array shows error, but the array remains active and the disk error shows as "green". Any thoughts? Error log attached.

    It was a disk error, SMART still looks good, run an extended test and if it passes you might give it a second chance, if it fails replace.

     

    You also have what looks like a cable problem with disk13, replace cables.
