Adding new hard drive causes existing one to not show

I have added a new 4TB drive to my system (parity is already 4TB). I am able to see the drive in "unassigned devices". However, I lost my preexisting drive that was already configured. 

 

They are going through an LSI 9211-8i which I flashed to IT mode. When I remove the new drive from the system, every thing boots up just fine.

 

What do I need to do to configure my new drive and not lose my preexisting drive?

First thing.  Add the new drive back in and boot up the server.  Then, go to    Tools    >>>  Diagnostics   and get a diagnostics file and upload it to a new post.  Give us the info on the missing HD-- Manufacturer, model and last four digits of the serial number.    Also provide us with the MB manufacturer and model number.  Also, check to see that you are using the latest BIOS for that MB.  (This sounds like an issue that a BIOS update might have addressed.)
