I noticed an I/O error for UnRAID boot USB flash card

 

Feb 12 16:29:37 Tower kernel: print_req_error: I/O error, dev sda, sector 15653872

 

does this mean flash card is failing?

    Personally, I would shut the server down.  Then, pull the flash drive and run a Chkdsk on it.  I would also checkmark the "Scan for and attempt recovery of bad sectors" option.
