woke up to issue with my server and need help finding out why.

Disk errors look related, make sure HBA is well seated, using latest firmware and adequately cooled, you can also try a different PCIe slot if available.

Put the HBA in a new pcie slot and rebuilt the parity drive.

Looks fine so far, not a single error with the HBA, you might also want to update the HBA firmware, since you're using a very old release.

Where do I get the update and how to?

Broadcom's support site, download the correct package for your HBA, instructions to update are in the zip.

I think I found it but scared i'm going to mess something up.

It look like I use rufus it make a bootable usb. Then put  mpt3x64.rom , sas3flsh.exe , SAS9305_24i_IT_P.bin on the flash drive. Then boot into the flash drive?

sas3flash -c SAS9305_24i_IT_P.bin -b mpt3x64.rom

I don't want to break my system

it should be:

 

sas3flash -o -f SAS9305_24i_IT_P.bin -b mpt3x64.rom

 

 

Well I tried with no luck. here is the .zip with the files from lsi.

 

What files do I all pull from it to put on the usb?

That error usually means you need to use boot with and use the UEFI flash tool.
