From the syslog in your Diagnostics, it appears that your LSI card is booting.  But as you can see the firmware version is out-of-date compared to what the vendors are currently providing.

LSISAS2008: FWVersion(11.00.00.00), ChipRevision(0x03), BiosVersion(07.11.00.00)

Here is the same line from my syslog:

LSISAS2308: FWVersion(20.00.07.00), ChipRevision(0x05), BiosVersion(07.39.02.00)

I am not an expert on this type of situation nor am I familiar with the reasons for all of the updates but I do recall that some were required because of operational issues.  Perhaps, @johnnie.black might jump in and provide some of his special insight.

EDIT:  I have two of these cards and this is the line from that syslog:

LSISAS2008: FWVersion(20.00.07.00), ChipRevision(0x03), BiosVersion(07.39.02.00)

I see several segfaults and call traces on the log, good idea to run memtest.

And yes, also a good idea to update the LSI firmware.

Yes, current problem isn't the LSI firmware, I just said it's a good idea to update since you're using a very old one.

There should be a board bios options to enable/disable option ROM for addon controllers, some have a general setting, others have a slot by slot setting.

ok i'm going to check that.

Question for you.

My system is in uefi. I saw in the usb key for unraid that there is an option  called:

Permit UEFI boot mode

Boot system in UEFI mode. Please check your system settings to support UEFI boot mode.

Should i checked the box ?

thank you
