All of a sudden out of no where the drive is connected to a USB 3.0 adapter and plugged into the 3.0 on the mb io

Unraid cannot handle an array drive being temporarily disconnected and then coming back and being allocated a different /dev/sdX type designation at the Linux level.   Your syslog shows this happened 

Jun 12 00:45:59 Tower kernel: usb 2-4: USB disconnect, device number 2

The fact that USB connections are prone to this behaviour is one reason using USB drives are not recommended for array drives.

Ok thank you I'll have to look into a pci adaptor or something
