Can anyone tell me if Disk5 id dead/dying?

Next time you should grab the diags before rebooting, but based on SMART disk looks fine, suggest you swap cables with another disk to rule them out if it happens again.

Ugh sorry about the reboot/diags. 

Well I have a rebuild going right now.

Interestingly, this is a new controller card and cables (less than a week old) and this is my first disk problem as well. Disk is only ~months old.

So if the rebuild gives problems, I will try connecting that port directly to the MB instead of the controller card and see what happens.
