Cleared Unassigned Drive does not have file system

I just added a disk to my array, and the disk says it is Unassigned and does not show a File System on it. I try to add it to a share or create a new share with the disk, and new shares are not created and the disk is not added to the old share. What should or can I do to get a file system on this drive?

It needs to be formatted, next to the array start/stop buttons.

I do not see that anywhere next to array start/stop button.

So I stopped the array and fired it up in Maintenance mode. Did a "Check Filesystem Status" and I got back this message:

 

 

xfs_repair: can't determine device size

 

What can I do about this? Is this a sign that the drive is shot? What other information is needed to diagnose this issue? I am new to unRaid and computer software in general. More of a hardware guy.

Disk wasn't added to the array because there were write errors, you can see them on the GUI, and would get a notification if enable, and since SMART doesn't look very good best to run an extended SMART test before trying to add it again, if it passes replace cables to rule them out and try again.
